Hintergrund:
Die chronische Herzinsuffizienz ist eine Erkrankung mit hoher Sterblichkeit und großer gesundheitsökonomischer Bedeutung, die für die Betroffenen gravierende Einschränkungen der Lebensqualität mit sich bringt. Zudem scheint die Prävalenz einer Depression erhöht zu sein, mit den Folgen einer herabgesetzten Compliance mit der medizinischen Behandlung und möglicherweise auch ungünstigeren Überlebensprognose. Obwohl die Lebensqualität des Patienten als wichtiges outcome-Kriterium der Behandlung gilt, existiert noch kein zufriedenstellendes Instrument für ihre krankheitsspezifische Erfassung. Darüber hinaus besteht noch Unklarheit darüber, welche Bereiche des Alltagslebens der Patienten besonders beeinträchtigt sind, sowie über Art und Ausmaß der Bedürfnisse der Patienten hinsichtlich Information, Beratung und Unterstützung.
Ziele und Fragestellungen:
Das erste Ziel der Studie besteht darin, einen aus den USA stammenden krankheitsspezifischen Lebensqualitätsfragbogen (Kansas City Cardiomyopathy Questionnaire, KCCQ), der in der Originalversion ausgezeichnete psychometrische Kennwerte aufweist, in einer autorisierten deutschen Version zu evaluieren. Ein weiteres Ziel besteht in der Erhebung der Bedürfnisse (needs) der Patienten im Hinblick auf Information, Beratung und Unterstützung. Auf der Grundlage dieser Bedürfnisanalyse soll eine manualisierte psychoedukative Intervention (Schulungsprogramm) entwickelt werden.
Studiendesign:
Die psychometrische Prüfung des KCCQ erfolgt nach dem üblichen Vorgehen der klassischen Testtheorie: Itemanalyse, interne Konsistenz, Retest-Reliabilität, Konstruktvalidität (Vergleich mit dem SF-36), diskriminative Validität (Vergleich von Patienten unterschiedlicher NYHA-Schweregrade), Änderungssensitivität (zweimalige Messung im dekompensierten und im rekompensierten Zustand). Je nach Ergebnis der Prüfung soll der KCCQ in einer vom BMBF geförderten Studie zur Ergebnisevaluation eingesetzt werden.
Für die Bedürfnisanalyse wird ein teilstrukturiertes Interview verwandt. Die erfragten Bedürfnisse der Patienten sollen Eingang in die Entwicklung eines Patientenschulungsprogramms finden, das in einem vom BMBF geförderten Projekt mittels eines kontrollierten randomisierten Designs evaluiert werden wird.

Quality of Life and Needs in Patients with Chronic Heart Failure
Background:
Chronic heart failure is a chronic disease characterized by a high modality, large economic burden and reduced quality of life of patients. In addition, the prevalence of a depression is high among this population with the consequence of lowered compliance with medical treatment and possibly reduced survival. Although the health-
related quality of live is considered to be an important treatment outcome, no satisfactory assessment instrument exists yet. Moreover, it is still unclear in which areas of their daily lives patients perceive strongest restrictions and what their needs are regarding information, counseling and support.
Objectives:
The study has two objectives. First, it aims at evaluating the German version of a new disease-specific quality of life questionnaire (Kansas City Cardiomyopathy Questionnaire, KCCQ). Second it aims at assessing the needs of patients regarding information, counseling and support. On the basis of this needs analysis a manualized psycho educational intervention program will be developed.
Study design:
The KCCQ will be psychometrically examined according to the traditional procedures, including item analysis, internal consistency, retest-reliability, construct validity (comparison with the SF-36) discriminative validity (comparison of patients in different NYHA severity classifications), sensitivity to change (two measurement points among patients with decompensated and recompensated state). Depending on the results of the evaluation, the KCCQ may be employed in another study evaluating an optimized disease management program. For the needs analysis a semi-structured interview will be carried out. The needs of the patients will be included in the development of a patient education program which will be further evaluated in the above-mentioned disease-
management project.
