Die Anzahl der Hüft- und Knie-Totalendoprothese (TEP)-Implantationen ist in den letzten Jahren stark angestiegen und demzufolge auch die Anzahl der Anschlussrehabilitationen. Ein großer Anteil dieser Patienten befindet sich im erwerbsfähigen Alter, so dass die TEP-Implantation oftmals Auswirkungen auf die erwerbsbezogene Leistungsfähigkeit und berufliche Teilhabe hat. Dies bedeutet für die Rentenversicherung, dass bei hoher Patientenzahl die postoperative Belastbarkeit der Gelenke im Hinblick auf die zuletzt ausgeübte berufliche Tätigkeit und auf dem allgemeinen Arbeitsmarkt sozialmedizinisch betrachtet und bewertet werden müssen. Die Prüfung von Maßnahmen zur Teilhabe am Arbeitsleben müssen ebenfalls beachtet werden. Die Inhalte und Strukturen des gesamten Rehabilitationsverlaufs müssen bei diesen Patienten vermehrt auf die Sicherung der Erwerbsfähigkeit und der beruflichen Teilhabe fokussiert werden. Eine besondere Herausforderung aus Sicht der Rehabilitationseinrichtungen ist, dass die TEP-Patienten häufig nicht rehabilitationsfähig aus der stationären Akutbehandlung entlassen werden. Ziele:

Hauptziel des Modellprojektes ist eine verbesserte berufliche Teilhabe durch innovative prä- und postoperative Leistungen bei Hüft- und Knie-TEP-Patienten.
Des Weiteren sollen Erkenntnisse gewonnen werden, wie die Zusammenarbeit zwischen kurativem und rehabilitativem Sektor verbessert werden kann, welche Compliance bei den Projektbeteiligten (insbesondere Rehabilitanden) für eine solche Prozesskette besteht und wie sich die veränderten Prozesse sowie Inhalte auf den nachhaltigen Wiedereingliederungserfolg auswirken.

Ablauf:

Die innovative Prozesskette besteht aus vier Phasen.
Bei der präoperativen Planungs-Phase handelt sich um eine Reha-Vorbereitung, welche in dieser Form in der Regelversorgung bis jetzt noch nicht umgesetzt wird. Der Patient wird auf die Operation sowie die sich anschließende Rehabilitation vorbereitet, indem er Informationsmaterial erhält sowie ein eintägiges Vorbereitungsseminar in der Reha-Einrichtung (Lahntalklink, Bad Ems) durchführt.
Die Akutphase beinhaltet den Aufenthalt des Patienten im Akutkrankenhaus mit Implantation der Hüft- oder Knie-TEP (Regelleistung).
Nach Entlassung des Patienten aus dem Krankenhaus beginnt die postoperative Übergangsphase (Prä-Reha), die der postoperativen Situation des Patienten (unvollständige Wundheilung, postoperative Schmerztherapie, eingeschränkte allgemeinkörperliche und spezifisch orthopädische Belastbarkeit etc.) Rechnung trägt. Es handelt sich hierbei um eine Reha-Vorbereitung, welche bis jetzt in dieser Form noch nicht umgesetzt wird. Ziel der Prä-Reha ist es, im Verlauf von ca. einer Woche eine umfassende Reha-Fähigkeit für die sich anschließende dreiwöchige Regel-AHB zu erreichen.
Nach Abschluss der Übergangs-Phase beginnt der Patient eine dreiwöchige AHB sowie gegebenenfalls eine Reha-Nachsorgeleistung (beides Regelleistungen).

Die wissenschaftliche Begleitung erfolgt durch die Deutsche Sporthochschule Köln.

Background: The number of total hip and knee arthroplasty (TEP) implantations has risen sharply in recent years and, consequently, the number of follow-up rehabilitations. A large proportion of these patients are of working age, so TEP implantation often has an impact on work-related performance and job participation. The examination of measures for participation in working life must also be taken into account. The contents and structures of the entire rehabilitation process must be increasingly focused on securing earning capacity and job participation in these patients. A particular challenge from the point of view of rehabilitation facilities is that TEP patients are often discharged from acute inpatient treatment not ready for rehabilitation. This increased treatment effort at the beginning of the AHB must then be compensated by the rehabilitation facilities. For a process chain from acute care to AHB and aftercare, which has an impact on rehabilitation therapy standards and patient-oriented treatment paths, there is a need for action especially in the interface communication between rehabilitation cost bearers, rehabilitation facilities and outpatient care. Procedure: The innovative process chain consists of four phases. The preoperative planning phase is a rehabilitation preparation, which has not yet been implemented in this form in standard care. The patient is prepared for the operation and the subsequent rehabilitation by receiving information material and attending a one-day preparatory seminar at the rehabilitation facility (Lahntalklink, Bad Ems).
