Through networking and joint action by the relevant institutional agents in Leverkusen, recipients of SGB II benefits who are addicted or at risk of becoming addicted are to be offered individualized health and social assistance in order to secure their social participation and enable them to access permanent gainful employment. On the one hand, this involves structural innovations in which the institutional agents relevant in Leverkusen are networked with one another in the form of a so-called production network (PNW). As the structuring centre of the model approach, the PNW develops binding, overarching service and support chains for people affected or threatened by long-term unemployment, people with addictions or at risk of addiction in Leverkusen, which can be transferred to other municipalities or regions. In the support chains, different sociopolitical services are brought together across legal districts (relevant here are the Social Code Book II, III, V, VIII, IX and XII) while maintaining the autonomy of all those involved in a transparent and coordinated, individualized support service for the target group. On the other hand, coordinators and developers for the new support service are created as professional innovations with the function of the integration scouts. These come from different institutions, but they plan and coordinate the developed support services together and implement them on a case-by-case basis, so that individualized assistance is guaranteed. There will be: - a networking strategy developed and implemented; Goal (smart): By mid-2020, the Leverkusen Production Network (GSWS) (incl. steering group) will provide comprehensive individualized health and social assistance for the target group of addicted persons or persons at risk of addiction who are entitled to SGB II benefits. - Functional positions of social scouts, health scouts and job scouts will be introduced and trained; Target (smart): By mid-2020, qualified integration scouts are working cooperatively and in coordination with the target group. Over the course of the project period (by the end of October 2024), 1,000 people in the target group have received an offer of overarching assistance and 500 of these are using one or more of the support services offered. - new concepts of cooperation developed; Target (smart): not applicable - results from the above two targets - comprehensive forms and offers of support tailored to individual needs developed; Target (smart): On the basis of generalized support chains tailored to individual cases, comprehensive assistance will be offered from 2020 onward, enabling participation such as access to permanent gainful employment. - professional concepts of the individual participants further developed; Target (smart): 2022 the professional concepts of the institutions involved in the production network are jointly coordinated and further developed for the specific requirements of this work. - a concept for housing placement for the target group and for accompanied habituation (also with support) is developed and implemented; Due to a concept for ensuring housing provision for the target group, in 20% of the cases that accept further assistance within the framework of the model project, termination of the tenancy is avoided or they are placed in a living space. - Access to the labor market enabled and supported; Goal (smart): 10% of those affected who are reached by the project and successfully participate in measures offered have entered employment by the end of the project period. A further 20% are in a position to be placed in gainful employment due to their health. - the concept of cooperation is extended to further areas; Target (smart): 2022 the developed and tested concept of the production network and its support chains is extended to the areas of extensive media consumption as well as migrants*. Further (smart) targets are: - By the end of the project (2024), a total of 300 people receive assistance in establishing a daily structure and support in the psychosocial area through targeted counseling and coaching. Of these, 60% will be able to participate in work trials and internships, which will prepare them for long-term professional integration. - 20% of people with an additional psychiatric illness (100 people) will receive appropriate assistance by the end of the project (2024). - By the end of 2024, the scientifically proven successful modules of the overall project will have been incorporated into a permanently funded structure within the jurisdiction of the AGL Jobcenter and the Leverkusen city administration. Likewise, these are incorporated into the learning circle of the job centers in the Rhineland and evaluated in terms of transfer possibilities. The project builds on existing structures and uses the specific experiences of the individual participants. However, the activities and the agents are largely "pillarized", so that a coordinated range of services is only available to a limited extent. The model project therefore cooperatively brings together the agents and their support services into a unified service for the target group.
