With its new organizational form and innovative measures, "Schwungbrett - Kräfte nutzen" offers a protected framework for testing development potential. Individual and tailor-made support - on the way to consolidated health and job prospects for people with permanent health restrictions. The cooperation between the Cologne Jobcenter, referral recipients and cooperation partners and thus the bundling of competencies is essential. For the participants, the centre of the project is increasing their basic digital skills for more freedom of action in everyday life as well as medical prevention and care. In the first phase, the focus will be on the first centralized occupational medical clarification and diagnostics to enable early intervention. In the second phase, in addition to increasing digital competence, permanent offers will be provided in a modular system. They will also be supported and empowered to achieve individual goals through regular presentations by experts. In the final phase, a realistic job perspective is developed with the participants and the application and integration process is supported. With the help of the network and a project diary, process control is gradually transferred to the responsibility of the participants and self-efficacy is increased. In addition, this will create maximum transparency and help beyond the end of the project. Insights are expected in the area of centralization and structural continuity options.
