Post-Rehab-Network

The aim of the project POST-REHAB NETWORK is the efficient and quality-assured care for patients in rehabilitation aftercare. The main focus is on care adapted to the changing ability of the rehab patient both at home and also in a work environment. The VIRTUAL PILOT "POST-REHAB-SERVICE", an online knowledge management tool for cross-player control of the network partners, is being developed to coordinate the services. The care process should thus also be made quality-conscious, transparent and smooth in the sense of "one face to the customer". The Service Network Duisburg, an association of health service providers for the Duisburg area, serves as a regional NETWORK MODEL. This Service Network currently comprises a health care supplier (Cohrs Duisburger Pflegeteam GmbH), a supplier of medical equipment (Sanitätshaus Oelrich), a pharmacy (Apotheke am Dellplatz) and as associate partners the Architecture Office Rüber, specialised in barrier-free construction, and a building company for the implementation and carrying out of the conversion work. The Network is furthermore supported by the North Rhine-Westphalian State Association for Ambulant Health Care Suppliers and the Health Association Cura-San. Assisted by the Chair for Industrial Sales Engineering of Bochum University and by the Chair for Labour and Vocational Rehabilitation of Cologne University, the PROCESS ORGANISATION of this Network will be analysed to optimise the cooperation of the network partners with regard to selected syndromes in an efficient and quality-assured manner.
