Many orthopaedic rehabilitation patients have got an unsecure perspective of return to work. In order to make a valid prognosis in the social medical judgement, a physician not only has to know a lot about the working conditions of his patient but to judge his abilities correctly. FCE systems as the IWS, which are designed to measure functional capacities objectively, are said to be helpful with this task. But the literature shows that its psychometric quality is not really confirmed. Especially the prognostic validity and the usefulness for social medical considerations still lack evidence.
We asked
1. about similarities and differences between the IWS results and the physicians' judgements of the patients abilities.
2. if the IWS contributes to a more realistic self-perception of the patients and their vocational perspectives.
Method:
We included 73 orthopaedic inpatients of the Klinik Muensterland, Bad Rothenfelde, the Therapiezentrum Brunswiek and the Therapiezentrum Friedrichshoehe, Bad Pyrmont, who had an IWS-evaluation after december 2004.
To answer the first question, we compared the IWS-result with the physician's judgement for each patient and searched for variables of the patient and the physician predicting the difference between the two judgements.
To answer the second question, we looked for changes in the patients' self-perceptions and their vocational perspectives, about which they told us in questionnaires they completed a day before and a day after the IWS.
Results:
1. The physicians thought the patients being able to do harder work in general as the IWS results indicated. Looking at the single abilities, the picture turned and the IWS indicated higher capacities as the physicians considered. This differences grew with professional experience of the physician and with the amount of occupational burdens of the patient which are not considered in the IWS (work in coldness, unconvenient postures,...)
2. About 2/3 of the patients told us to have corrected their self-peceptions, this result corresponded with changes in the PACT. The number of patients whose PACT index was the same as the IWS results, rose from 21 to 40. The vocational perspectives stayed the same in most of the patients.
Conclusions:
1. Especially when patients are difficult to judge, the IWS may be helpful for social medical considerations. But because of the test-situation not being completely realistic, its validity is suboptimal and its results have to be interpreted and adapted to the individual working conditions and the medical prognosis of the patient by a well-informed and experienced physician.
2. The IWS let the patients lern to judge their own abilities in a more realistic way, but the occupational plans depend not only on the perceived own capacity but on a lot of different variables outside the patient, as the labour market for example, so that it kept much more stable than the patients' self-perceptions.
