FaKT - Case management in the context of reduced earning capacity: Participation-based support for insured persons returning to work

The aim of the intervention is to reintegrate more insured persons with temporary reduced capacity to work into the labour market. The FaKT model project is aimed at (younger) insured persons with oncological and neurological diseases who are highly motivated to return to work. The target group should be reached as early as possible - already in a rehabilitation clinic or when applying for an EMI pension for the first time. During the recovery process and after returning to work, participants will receive personalised and comprehensive support from case managers with individually tailored services. In order to enable them to take up employment subject to social security contributions without immediately losing the financial security provided by the EMI pension in the event of failure, entitlement to the full EMI pension should continue for a further 12 months in parallel with employment.
