Steering with the rehabilitation compass: everybody in the same boat. Interface management for the early detection of addiction rehabilitation needs and timely navigation towards addiction rehabilitation

Description / Topics

Aims:
Early detection of (incipient) addiction problems and their management through preventative measures and addiction rehabilitation.

Background:
Oftentimes, people with addiction problems seek or find help quite late in the course of their disease. Thus, otherwise effective preventative measures and medical rehabilitation are not as successful in preserving and restoring social and occupational performance and participation. Accordingly, there is a high need for a low-threshold service which supports people with addiction problems and their environments to detect rehabilitation needs and helps paving the way for preventative measures or medical rehabilitation.

Content:
Relevant social, organisational, and medical structures will be empowered to recognise and address addiction problems in their clients, co-workers, and patients. These will contact a “navigator” who will arrange for them to participate in the “rehabilitation compass”, an instrument to assert prevention/rehabilitation needs and to facilitate the according application process.

Target population:
300 people with (incipient) addiction problems living in Oldenburg or Bremen.

Evaluation:
The innovative measure will be rolled out starting October 2020. A process and outcome evaluation will determine its effectiveness.
