Gesundheit4PunktZukunft - Preventive Health Update for Families

The project focuses on the "family system". Both children with illnesses and parents with illnesses can throw this system out of balance in the short and long term, even leading to lifelong socialization away from the world of work. Prevention is therefore of the utmost importance in such systems. This applies to "classic" families as well as single parents, patchwork families or same-sex parents. The aim of the project is to increase health skills in 200 of these family systems burdened by illness in order to improve social and job participation (strengthening the principle of "prevention before rehabilitation"). An essential prerequisite for this is the expansion of the counseling competencies of the employees of the job center and the youth welfare office by testing innovative, cross-jurisdictional cooperation based on a systemic approach. On the basis of multiprofessional teamwork, the project offers group and course programs as well as an open contact point for families in receipt of SGB II benefits. Another focus is the development and testing of offers designed by the participants themselves in order to achieve a better fit and thus a higher acceptance. Furthermore, outreach coaching takes place in the social and living environment of the families. This is deliberately network-oriented and cross-jurisdictional. Digital consulting and support services round out the portfolio.
