The digital platform is called aktivAPP and will be enlarged in the already existing case work and profilings within the Jobcenters. It will measure the "rehaScore", which indicates an individual index (of each person) of possible danger to diminished employability. Based on the rehaScore, action strategies should be derived. The customers should receive supporting measures for risk avoidance by offering existing measures with focus on the specific needs and requirements. In addition, new performances will further evolve to cover the services for groups and individuals.
As a second tool, expert conferences will be held with the objective to unite professionals from relevant areas for interdisciplinary cooperation (expert pool). The central role is given to the medical professionals. They are to raise awareness of the connection between employability and health for other partners.
Following the results of the aktivAPP and the conferences, action strategies will be extended and new strategies will be developed. This will be done through collaboration with the professional network and the participation of joined partners.
The participation in the project and use of aktivAPP is entirely voluntary. It is offered to all individuals receiving Social Security Code (SGB II) benefits without any relation to sanctions or reduction of benefits.
What makes project KOPF22 innovative?
What are the major risk factors and influences on the working ability of people receiving SGB II benefits? With KOPF22, the participating Jobcenters want to pursue this question in a structured manner for the first time within their organizations.
The innovative value of the project comprises the exploitation of digitalisation opportunities as the decision-making assistance through self-learning algorithms (within aktivAPP).
The development and use of a digital platform to enlarge the first profilings with the aim „prevention before rehabilitation“ represent the structural and operational refocusing in the Jobcenters.
Collaboration with the experts of different relevant disciplines and areas contributes the interlinking of those systems and raises awareness for common themes and challenges and thus strengthens the expertise of all the participants.
Target group:
Target group of project KOPF22 extrapolates from the funding guidelines of rehapro:
- individuals with expected or commencing rehabilitation needs
- individuals with mental impairments
- individuals with dependency-related illnesses
- individuals with complex need of health assistance
Due to multilevel structuring of the project, the target group has to be differentiated accordingly. The use of aktivAPP will be first offered to new clients of the participating Jobcenters. Then it will be expanded to all customers who already receive benefits. After the individual rehaScore have been specified, the focus will be on those showing a specific score of possible danger of diminishing working ability (partially or completely). At the same time, it is so low as this risk can be prevented with help of other instruments of the project (action strategies and professional conferences/expert pool).
Funding policy objectives:
Introduce innovative services/methods to maintain or recover the working ability, or to prevent the threatening (partial) reduction of working ability.
