The Erzgebirgskreis job center would like to set up a regional, digital and outreach contact and coordination point. The network should consist of as many actors as possible who are active in the fields of health, prevention and rehabilitation. This interface can, with all actors in the network, professionally from a single source in the health and rehabilitation system identify people with mental impairments or illnesses at an early stage and advise and control them individually and according to their needs. In the Erzgebirgskreis job center, the number of cases confirms a continuous increase in the number of people with mental health problems and illnesses. Also due to Covid-19 and the infrastructural problems in the Erzgebirge there is an urgent need for a simple and above all cross-legal offer for those affected. The professionally qualified health and rehabilitation coach is able to recognize mental illnesses / impairments and to speak to the person concerned. As an independent guide, you will be forwarded to a network partner with constant support from the health and rehabilitation coach. For example, a clinic in the region can carry out a specialist check-up and determine the need for prevention or rehabilitation as well as the motivation of the person concerned. The aim is for citizens to improve their professional participation by being able to switch to a profile that is closer to the labor market, because the state of health has demonstrably improved and, if necessary, they can resume work for at least 6 hours a day. In the course of individual participation, innovative individual services and measures can be implemented in regular business.
